Considering Leaving My Family And Going Into Foster Care?

I'm 13 and my family is crazy, my grandmother smokes and is always bitchy and my mother is always threatening to put me in mental hospitals, my grandmother always says she is going to move out and everyone is always yelling at me. No one in my family has finished high school accept my sister who is mostly the center of attention. My brother has well... problems with everything. And I used to cut and am starting again. My mom threatened to put me in a mental hospital if I didn't see a therapist and I want to go into foster care, to live with a good family who cares and helps. What can I do to get into foster care and what happens when you go into foster care? Can I get the details please? Just so I know what is going to happen to me if I do leave?
